• Please ALSO fill out this paperwork if being seen for headache or facial pain. 

Step 2: Ask your referring doctor to send us any pertinent notes and test results.

We are closely affiliated with the RRHS and URMC, but since we are a private practice, we may not have direct access to prior medical records.  It is important that you or your doctor provide us with this information, ideally prior to or at the time of your visit.
